3) save the completed form, then
4) email it as an attachment to info@fortyplus.org.nz

If you would prefer not to use a computer to fill out the form:
1) print your own copy of the blank form
2) fill out the form by hand
3) fax it to 377 2181; post it to PO Box 13-026, Christchurch 8141; or deliver it in person to the 40+ Employment office, level 5, 141 Hereford Street



or if you'd rather:

• phone us and ask to have the form sent to you.

Notes:

A question in the form asks if you would like 40+ Employment to share details of your vacancy with other agencies.

• Choosing YES means that we will give details of your vacancy to other not-for-profit organisations associated with employment who may themselves then advertise the position to their own clients. Any interested clients, however, would be referred to 40+ Employment for our approval though they would not necessarily be over the age of 40.
• choosing NO means that any applicants will be registered with 40+ Employment, and will be therefore be over the age of 40 and known to 40+ Employment staff.

Payment method (see fee structure page for more detail):

• VIP scheme means that for an upfront donation (we suggest $75), 40+ Employment will advertise your current vacancy - and any further vacancies - for the current calendar year
• Placement fee means that you will be invoiced for an amount equavalent to 1% of the employee’s gross annual salary/wage + GST, or $100 + GST – whichever is the greater (temporary employment is by negotiation) - but only when one of our jobseeking clients is employed by you. This fee is payable per client recruited
